Healthcare Estate Repair and Maintenance

The National Health Service is sitting on a repair bill it can no longer ignore. The cost of clearing the maintenance backlog across the NHS estate in England reached £15.9bn in the 2024 to 2025 year, up almost 16% in twelve months, with a large share of it classed as a significant or high risk to running hospitals safely, according to NHS figures. Buildings are failing faster than they are being fixed.
Money is beginning to follow the problem. The UK Government has committed at least £9bn a year to critical maintenance across health and other public estates, rising over the next decade. Much of that work will not be done by the health service itself but passed to outside specialists in the safety critical trades, the firms that keep fire systems, water hygiene, ventilation, medical gas and electrical systems tested and working. A hospital, a care home or a clinic cannot let any of these lapse, whatever the state of its budget.
That is what makes companies working in the sector appealing to acquirers. The income is repeat business, it is underwritten by public money and protected by regulation. Buyers who built their platforms in social housing and general building safety are now hunting specifically for firms with real experience in healthcare settings, where the standards are higher and the door is harder to get through.
The owners of these businesses are in an unusually strong position, because a firm with the right accreditations and a clean record in clinical settings is scarce. Buyers, for their part, have to be sure the quality is genuine, since a compliance failure in a hospital carries non-financial costs.
Demand tied to the health estate should hold up whatever the wider deal market does, because the buildings will still be standing and the rules will still apply. In a patchy year for transactions, that kind of certainty is rare, and it is why this quiet corner of healthcare is starting to draw attention.
